The official nomination form should be used to nominate an individual for consideration for any category listed above. Please note the detailed description of each award and the nominating criteria listed for each category:
ATHLETES WITH DISABILITIES HALL OF FAME
Hall of Fame nominees MUST meet the following criteria:
- Must have competed as an athlete with a disability for a minimum of ten (10) years, including five (5) years as a Michigan resident;
An individual must have participated at the local, regional, state or national level as an athlete with a disability in competitive athletics. An individual should have ongoing involvement in the sport(s) of his/her choice and have demonstrated achievement and accomplishment in the sport(s). His/her participation should be representative of good sportsmanship and serve to enhance the sport(s) for individuals, his/her team or the sport overall.
The individual should be able to demonstrate participation in community awareness programs and/ or events, involvement in civic and/or volunteer organizations, community service organizations, camps, and have impacted the community at large, or show impact to benefit the community via other activities. The nominee should have character and integrity that exemplifies a positive role model in the community.
- Nominations may be submitted for individuals who are deceased;
- Must have been a Michigan resident for at least five (5) years;
- An individual can only be honored once in this category.

RICK KNAS LIFETIME ACHIEVEMENT AWARD
The Lifetime Achievement Award recognizes an individual who has made significant contributions to athletes with disabilities in the State of Michigan.
Lifetime Achievement Award criteria includes the following:
- Must have made significant contributions to the disabled athletic community;
- Must have impacted the community that serves Michigan athletes with disabilities for a
- minimum of ten (10) years;
- An individual need not be disabled, nor an athlete;
- Must have been a Michigan resident for at least five (5) years;
- Nominations may be submitted for individuals who are deceased;
- An individual can only be honored once in this category.

ATHLETE OF THE YEAR AWARDS
Each year, the Hall of Fame recognizes a Male and a Female Athlete of the Year. These awards recognize the recent achievements made by the top athletes with disabilities in the State of Michigan. Nominations for teams may be accepted under Athlete of the Year Awards. First consideration will go to the athlete.
Athlete of the Year Award criteria includes the following:
- Must have competed as an athlete with a disability;
- An individual must have participated at the local, regional, state or national level as an athlete with a disability in competitive sports;
- Must make his/her primary residence in the State of Michigan.
Please also note that:
- An individual may be nominated posthumously for achievements made in the past year;
- Voters may take into account the nominee's additional athletic successes throughout his/her career, but the award is primarily predicated on the success achieved in the last year;
- An individual can be honored with the Athlete of the Year Award on multiple occasions.

MARK "DOC" ANDREWS RISING STAR AWARD
The Rising Star Award recognizes an athlete who has competed as an athlete with a disability and exemplifies determination and achievement.
The Rising Star Award criteria includes the following:
- Must be under the age of twenty five (25) at the time of nomination;
- Must have been a resident of the State of Michigan for at least five (5) years;
- Must have competed as an athlete with a disability;
- An individual must have participated at the local, regional, state or national level as an athlete with a disability in competitive athletics.
- An individual should have ongoing athletic involvement in the sport(s) of his/her choice and have demonstrated achievement and accomplishment in the sport(s). His/her participation should be representative of good sportsmanship and serve to enhance the sport(s) for individuals, a team, or the sport overall.
- Must have served as a positive role model for the community;
- The individual should be able to demonstrate participation in the following areas: community awareness programs and events; involvement in civic, volunteer, and/or community service organizations; and sports camps. As a result of participation, the nominee should have impacted the community at large or show impact and benefit to the community via other activities.
- The nominee should demonstrate character and integrity that exemplifies a positive role model in the community. This involvement might be in school programs, clubs, church activities and other community service groups.
- An individual can only be honored once in this category.

- When submitted, the official nomination form should be accompanied by a current photo of the nominee, a detailed summary of their athletic and community involvement, and a minimum of two letters of recommendation. Nomination forms and all supporting materials (the "Nomination Package") must be submitted at one time. Once the Nomination Package is received, reviewed and deemed complete, no submitted materials will be returned.
- Any Nomination Package deemed "Incomplete" will be returned in its entirety to the party who submitted it with a checklist indicating the missing component(s).
Nomination forms will be available online for convenience, but nominations must be submitted with supporting materials via U.S. mail or other in-person delivery.
The Nomination Package, including the official nomination form and any supporting materials must be received by "...DEADLINE TO COME", via U.S. mail or in-person delivery.
